Your access and use of the Platform, any part thereof, or anything associated therewith, including its content (“Content”), any information, products, or services provided through the Platform or otherwise by Company or its contractors, and any affiliated website, software, or application owned or operated by Company (collectively, including the Platform and the Content, the “Service”) are subject to this Privacy Policy unless expressly stated otherwise.

We are committed to respecting the privacy of users of the Service. We created this Privacy Policy to tell you how Company collects, uses, and discloses information in order to provide you with the Service.

Protected Health Information

What information does Company collect?

When you use the Platform and communicate with Company personnel by phone, video, email, text message or other means of communication, we may collect personal information including, but not limited to, your name, email address, phone number and location. We use this information to facilitate delivery of healthcare services, and to supervise, administer, and monitor delivery of such services.
